What to bring to your exam (first time patients):

• All current glasses (including computer glasses, reading glasses, sunglasses, etc)
• A list of all medications you are taking

If you wear contact lenses (all patients):

• Your contact lens prescription or your contact lens specifications (boxes/vials/labels) if available
• Current glasses and sunglasses
• Please wear your contact lenses to your appointment unless you are having problems with the lenses

*If you have an illness/cold or think you might be on the verge of one, please be so kind as to reschedule.

**Missed Appointment Policy Notice:

Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Please kindly give 24 hours notice if you need to reschedule an appointment to avoid a missed appointment fee. Thank you for your understanding and cooperation as we strive to best serve the needs of all of our patients.
