2. Current glasses and sunglasses
3. Please wear your contact lenses to your appointment unless you are having problems with the lenses

**Missed Appointment Policy Notice:
Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Please kindly give 24 hours notice if you need to reschedule an appointment to avoid a missed appointment fee.
